- System brand and model numbers
- Fault code and affected unit
- Problem description and timeline
- Whether the issue is new, recurring, or intermittent
Field data and attachments
- Photos of nameplates, wiring, boards, and controllers
- Voltage, resistance, pressure, and temperature readings
- Controller screenshots or service tool data
- Work already performed
- Parts replaced or settings changed
